You must include your PEBB-assigned benefit number on your claim form, or your claim will be delayed, or possibly not processed, as this is the only number that ASIFlex uses to track your account.  If you don't know what your PEBB-assigned benefit number is, there are several options included below that can assist you with locating this number. 

Remember, if you don't include the correct number on your claim, it is possible you won't get your money back!
